ELIGIBILITY

Eager to add your voice for positive change? Read below to see how to determine if you're eligible! 

 

Starting October 1, 2024 we will be surveying youth who have birthdays between October 1, 2005 and September 30, 2006 and took the survey when they were 17 years old.

 

We reach out to eligible youth, mostly by phone, using a variety of avenues of contact, such as: caseworkers, group homes, foster parents, biological family, and direct contact.

Taking the survey is really easy. Just look to the right to find your options. You can either take the survey online, print a copy to mail or fax back to us, or you can call us to do the survey over the phone. There isn't always an interviewer working at all hours of the day, so if no one picks up, leave us a message and we'll get back to you ASAP!

 

The survey only takes five minutes. How else can you spend just five minutes to make a positive impact on the whole foster care system?

Have I mentioned how much we love hearing from youths? Because you have so much going on in your lives at this age, we need your help staying in touch! Reach out to us to make sure we know how to contact you.

 

If your contact information changes, give us a call or send an email to let us know. New phone number, email address, Facebook page, LinkedIn profile, or mailing address, etc. If there's a good way to get in touch with you, we would love to know about it. We've even gotten an Xbox Gamertag or two!

If you filled out a paper survey, you have a few options for returning it. 

 

  • Scan or take photos of it and email it to mandi.breen@ocfs.ny.gov

  • Fax it to (518) 442-5732

  • Contact us and request a prepaid envelope be sent to you
